Patch builder prepopulation issue

Resolved - The issue with Patch templates not prepopulating Patch creation forms has been fully resolved. Monitoring - Our Engineering team has confirmed that some frontend changes caused this issue, and has implemented a fix. Users should now be able to use Patch templates as expected.

We are monitoring the results to ensure the issue is fully resolved.

Investigating - We're investigating an issue where Patch templates are not pre-populating Patch creation forms as expected.
